The applicant seeking for accreditation shall be required to submit the following documents:

1. Duly accomplished application form;

2. Certified true copy of the latest Mayor’s Permit to Conduct Business or Municipal Business License;

3. Certified copies of the applicant’s Certificate of Registration duly issued by the Department of Trade and Industry/Bureau of Trade Regulation and Consumer Protection (BTRCP);

4. In the case of corporation or partnership,

4.1.1. Certified true copy of the Articles of Incorporation;

4.1.2. By-laws, or Articles of Partnership and amendments thereof, duly registered with the Securities and Exchange Commission (SEC);

4.1.3. Business Name Certificate;

4.1.4. Resolution of the Board of Directors of the Corporation entity authorizing the filing of application and designating the person authorized to sign and act for and in its behalf and transact business with the PTC;

4.2 In case of single proprietorship

4.2.1. Business Name Certificate and amendments thereof;

4.2.2. Letter of Authority from the owner;

4.3 In case of cooperatives

4.3.1. Registration Certificate from the Cooperatives Development Authority;

4.3.2. Letter of Authority from the cooperative;

5. Vicinity/location map and site development plan;

6. Applicant’s latest income tax return and audited financial statement;

7. For individual service providers;

7.1 Proof that the applicant had passed a training program conducted by recognized pertinent institutions/agencies authorized to conduct the same (DOT for tour guide, PNRC for life guards, etc.);

7.2 Certificate of good health issued by any accredited government physician;

7.3 Police clearance;

7.4 In case of alien applicant, valid visa from the Bureau of Immigration and proper work permit from the DOLE;

7.5 Mayor’s Permit/Municipal License;

7.6 Latest Income Tax Return

8. Other documents as may be required by PTC;

The following documents will also be required:

b. For projects located in environmentally critical areas/tourist zones, copy of Environmental Compliance Certificate (ECC), or Certificate of Non-Coverage (CNC);

c. Endorsement by the local stakeholder (e.g., local community) where a particular tourism project is being (or will be) implemented. If such would be difficult or almost impossible to secure, the applicant must show proof or evidence that an attempt was made and that an information dissemination and corresponding public disclosure about the project was undertaken.
